Admission Application Missing Documents

Eligibility Verification

U.S. Students
To verify your eligibility into our School of Nursing you must submit one of the following:

A current copy of your wallet-size license to verify professional status in your appropriate health care profession. BRN or MRN applicants must also submit an official transcript showing the completion of Associate Degree nursing education or RN Diploma education.

An official AARTS, SMART, CCAF, CGI, or DD214 transcript.

International Students
You must submit an official transcript that reflects graduation from a school of medicine with a minimum GPA equivalent to a 2.0 or higher on a U.S. scale. You may use either of the following evaluation agencies:

Educational Credential Evaluators (ECE) in a Subject Analysis format.

World Education Services (WES) using a Course by Course evaluation format. Note: If you have had an evaluation completed by WES and did not specifically note it was needed for Excelsior College, you must request an update to your evaluation by WES prior to submitting it to us.

Pre-Admission State Board Form

Your application indicates that you reside in a state where there are additional stipulations for Excelsior graduates and you must complete and submit a Pre-Admission State Board Form.

This form includes an acknowledgement that you understand the additional requirements for the state where you currently reside and/or where you plan to apply for RN licensure. This form must be submitted to complete your application. Download your state-specific PDF form from this table. Complete the form, fax it to 518-464-8833, or mail it to:

Transcripts

Please submit the official transcripts, examination score reports, and/or military documentation you listed on your application.

Official transcripts and documents are those that a college, testing agency, or education office sends or gives to you in a sealed envelope which you then forward unopened to Excelsior College. Some sources may not send official transcripts/documents directly to you. In that case, have the source send the documents directly to Excelsior College's Office of Registration and Records:

Please remember:

Official transcripts must:
- Have an issuance date of six months time or less.
- Include the official seal and registrar's signature (signature is usually a stamp)
- Include 2 forms of student identification (e.g. social security number, date of birth, current address)
You must provide us with the name by which you are identified on your official transcripts.
Do not send unofficial copies. We will not evaluate unofficial copies, and submitting them will delay the application process.

If you have military credit from AARTS, SMART, CCAF, CGI, or DLI/DLPT please contact your branch on how to send your transcript.
